1. Order a Repeat Prescriptions
All patients on long-term medicines should try to be seen, at least, at 6 monthly intervals. A repeat prescription will not be issued to any patient who has not been reviewed by the doctor in the practice in the last 6 months.
For patients with GMS medical cards:- Please go to our Front Reception Desk for your Repeat 3 monthly Prescriptions.
If you have a prescription from a hospital this needs to be reviewed by the doctor and a prescription issued from us at the Rowe Creavin Medical practice.
24 Hours notice is needed for all Repeat Prescriptions. This allows the doctor to check it and ensure your records are updated accordingly.
When making an appointment please let the receptionist know if you require treatment over and above the normal doctor/nurse consultation time. e.g. vaccination, smear, coil, Be well medical review.
Appointments are also available for specialised services eg. coil insertion, weight management clinic, cardiovascular screening, Well Person checks, Dexa Scan.
There will aways be a doctor available to see emergencies in our Urgent care /walk in clinic.
A consultation may overrun for example with elderly patients or emergencies. Please be patient with us, as you maybe the patient requiring the extended consultation.
If you cannot attend a scheduled appointment please call the surgery on 051 370057 as soon as possible to cancel. This allows us to slot in another patient in your place.
What hours are we open?
Office Hours : 9am - 5pm Monday - Friday. We also provide emergency cover between 8am to 9am and 5pm to 6pm Mon - Fri. Outside these hours please phone CAREDOC on 1850 334 999
Emergency telephone calls will always be dealt with by a doctor.
House bound patients and patients in a nursing home will always be accomodated with a house call.
Home Visits are best arranged before 10 a.m.
6. When will I get my Blood test or X-ray results ?
All Blood results arrive at the practice electronically from the labatory department at Waterford Regional Hospital. Most test results are best discussed at Follow-up appointments, in person. It is the responsibility of the patient to phone for his or her own results. X-ray results take up to 10 working days to come back to the surgery. Normally results will be back within 2 weeks.
If your GP decides that a telephone follow-up is suitable:- Test results are available from the Practice Nurse. Please telephone 051-370057 Patients are asked to leave their name, date of birth and phone number so that their Nurse can contact them with their results. Cervical Smear Test results can take up to 8 weeks to come back from the lab.
7. How do i get my Form Filled in?
If you require the Doctor to write a letter eg. Housing Letter, or to fill in any other forms such as driving licence, school college form etc. please allow 3 working days for completion.
There is an administrative fee to be paid when collecting your completed Form or Letter from reception. For most forms, the standard 'form fee' applies. However some forms involve greater administration or medico-legal responsibilities and a higher fee applies.

8.When do I pay for my medical services?
Payment is necessary at each medical visit. We do not offer credit.
Keep your itemised receipt for tax purposes. Back dated receipts can be issued for tax purposes however this service will incur an administrative fee. Full price list of medical services displayed at reception noticeboard.
How can I pay ?
For your convenience, we accept: Cash, Cheques, LASER, VISA, MASTERCARD.
Health expenses are tax deductable. To claim relief you will need to keep all receipts and complete a MED 1 Form available from revenue website.
Do Insurance schemes (like VHI /Quinn/Hibernia) cover your GP services ?
Certain procedures are covered by insurance schemes. These services are Cryotherapy, joint injections, blood letting , suturing, Removal of Moles, and IGTN which are covered by health insurers. If you wish to avail of these services under private insurance, please bring your medical insurance scheme details with you to the reception desk on arrival. Routine consultations and other services are not covered by your health insurance. Contact your insurance company for more details. See Useful links on this website for all health insurers.
Patients with a valid medical card are covered.
9. How to apply for a Medical Card
Medical card application forms can be obtained from reception. Patients are required to complete the application form and return it to the receptionist. The form will then be signed by one of the GP partners. New patients will be requested to attend for consultation with a GP partner prior to submitting application for a medical card.
Existing Rowe Creavin patients can leave completed medical card application forms at reception to be signed by one of the partner doctors and collected the next day.
The medical card application form will then be sent by the to the HSE, Community Care, Cork Rd, Waterford where their application will be processed.

Services Provided to Medical Card Holders
The GMS medical card entitles holders to a wide range of medical services from their nominated GP / surgery. These services are provided to the medical card holder free of charge and include standard consultations for treatment of illnesses and related services necessary for the treatment of a wide range of medical conditions.
Full medical card and Doctor Only medical card holders are entitled to the same services free of charge within the surgery.
Services Not Covered by Medical Card Entitlements
The Medical Card Scheme entitles current card holders to a wide range of services from their family doctor. The doctor receives payment from the State for these services.
There are, however, some services which your family doctor may be able to provide but which are not paid for by the State. The most usual services requested which are not paid for by the State are:

EXMINATIONS RELATING TO INSURANCE POLICIES
EXAMINATIONS RELATING TO FITNESS TO DRIVE INCLUDING EYE TEST
PRE-EMPLOYMENT EXAMINATIONS
SCHOOL ENTRY EXAMINATIONS
EXAMINATIONS IN CONNECTION WITH FITNESS TO TAKE PART IN SPORTS
SOME VACCINATIONS (INCLUDING TRAVEL VACCINATIONS)
SOME FAMILY PLANNING SERVICES
SOME SCREENING TESTS
GMS Medical Card patients requesting any of the above services are responsible for the relevant fees.
